Augmentation of Vocal Cords by Autologous Fat

Overview
Vocal fold augmentation is a minimally invasive phonosurgical treatment option for glottic insufficiency. The most common symptoms associated with glottic insufficiency are dysphonia, vocal fatigue, odynophonia, compensated falsetto and dysphagia. Vocal fold augmentation can be effective at closing glottal gaps less than 3mm. The autologous fat belongs to long lasting injection substances used for this procedure.
The outcomes of autologous fat vocal fold augmentation in 9 patients are presented. The glottal gap was 1-3mm preoperatively. During the procedures average 0.9ml of fat was administered into the vocal fold, minimally 0.5ml, maximally 1.5ml. Except for one man the total closure of vocal folds with a closed quotient 5-30% was achieved postoperatively. In 8 out of 9 patients the MPT was extended – by 3.8 (± 3.2) seconds average. In 7 out of 9 patients there was a significant subjective voice quality improvement according to Voice handicap index questionnaire. The average overall Voice handicap index decreased from 74 (±26.2) points to 20.9 (± 23) points, i.e. by average 53.1 (±39.2) points. All the monitored parameters of the objective voice analysis improved on average.
