Overview
Pemphigus vulgaris is an autoimmune bullous disease affecting skin and mucosae covered by squamous epithelium. The first signs of the disease appear on oropharyngeal mucosa and can precede the main dermal symptoms by months or years. The symptoms in the oral cavity can imitate mycosis or aphthous stomatitis, with no response to standard treatment. If not giving a thought to pemphigus, this can put off the right diagnosis and the treatment and can lead to suffering of the patient. The treatment should be managed by the dermatologist and includes administration of corticosteroids and immunosuppressive drugs. The case of pemphigus vulgaris in a 78-year-old woman is described, the resemblance between erosions in the oral cavity and the oropharyngeal mycosis led to postponing of the diagnosis till the typical dermal morphs had appeared.
